What were the worst trading mistakes that we commited from the list?

You can add one or two more points. averaging and overleveraging to losing position and selling in panic. Converting intraday or swing trades into short term and long term.

I have commited all the mistakes mentioned by you plus these mistakes also.

On mistake in trading I can only aDD that I had done so many mistake that now if i start counting and try to rectify it It will take my whole life many of them I still dont know and try ing to find out
