Has this happened to you...?

#1
I applied for an a/c with Indiabulls. After 2 months the rep comes back and tells me that there is a signature mismatch and asks me to fill out another DMAT opening form. He tells me to just sign and he'll fill in the details.

Shouldn't Indiabulls return my old form? Is there a chance of it being misused? Is this a fraud to open benami DMAT accounts?

Has anyone had this experience before?
Thanks
Jay

If there is minor mistmatch in signature then ask you bank to verify your signature on "Signature verification of individual" format. Don't forget to ask your bank to stamp (round stamp) on it. Give this form to your broker.
