demat of stokes

Hi, I hold 3 shares of a xyz co. in physical form. These are in my and my wife's name.
The demat account i have is only in my name " vijai kumar panchal ' whereas the spelling of my name is " vijay kumar panchal " in the certificate.
Pl. suggest the procedure to convert these to electronic form in my present demat account.

fwd alongwith affidavit signed by notaray that both are same person
