I suppose its related to PAN irrespective of DEMAT!

pan card copy is not required for any amount. it is because the only identification which is verified is pan no alone. PAN no will be taken from your demat account and hence even if a person applied by giving different demat account nos he cannot get the allotment. the registrar will reject.
the address given in the form does not matter and all bank payment- refund - details will only be as given in your demat account. beware before entering into complications. there are lot of ways to make money.
