PAN Card for NRIs

I've seen several newspaper articles recently that talk about the new rules making it easier for NRIs to apply for a PAN card - copy of passport and of a local bank statement are all that are required. Does anyone know where I can download an application form? I went to the websites of UTI Technology Services and National Securities Depositories but couldn't find this form.
