Dividend % is the % of dividend declared by the company. Say 15% dividend declared means, if the face value of the scrip is Rs.10/-, you will get Rs.1.50.
Dividend yield is the yield you will get on your invesment. that is dividend divided by market value of the scrip expressed as a percentage. In the above example, suppose the market price of the scrip is Rs.15/-, dividend yield will be 1.50 / 15 X 100 = 10.

In short, Dividend percentage is the dividend declared on the face value of the share, whereas dividend yield is the return you are getting out of your investment or market value.
