Transfer Of Shares From ICICIDirect To IndiaInfoline

I want to transfer my shares from icici demat account to india infoline demat account. I know that I have to fill up the IDT form given by icicibank because the transfer is between NSDL to CDSL. I want to know how I have to fill it up?I know that I have to give the "Target id","ISIN No","Name of security and quantity". There are other optoins to enter "Market Type", "Settlement No.","Settlement Details" etc. Do I need to fill up these things for "off market transfer" or not? Can anybody help me? If anybody can show me a preview that will be easy to understand.

You have to fill ONLY off-market transfer. I will try to show you a sample
