It can either be bad-ticks, no activity (dummy ticks) or a refresh issue within the quote provide. No - because, it is perfectly possible that the high and low are range bound. Especially, if the volume is low or insignificant it is possible that some one is holding the low and the high, before the big drop that is comes next. Those moves, I have seen before. What does the volume say ? Is it changing or is it stagnant ?

Yes, it is a heikin-ashi chart.
